Why is it ok for one and not the other?

My brother - a lecturer in a university - commented to a female work colleague about his having to visit a doctor about some problem 'below'. There is nothing between them at all, but his girlfriend did not like it at all. Why do some women think they can speak about heavy periods to other women, but he can't mention his problem?
