How do I properly report an honorarium to the IRS and my state department of taxation?

I was paid an honorarium by check for a speaking engagement in 2008, and I have no doentation of it. Since there is nothing like a W2 for it, how do I properly doent it on my federal and state (NY) tax return? Thanks!
